There is no middle ground with me, either.
And I do enjoy a Pop-Tart!

Having a change of clothes in the car is a must. Inevitably something gets spilled, or worse! During an all day outing.

Pop tarts are awesome!!! I think not having a clear cut plan is a bad idea. Plans always change a little so I need to be a little flexible but at least a general idea is a must.

Like others, not having enough snacks and drinks is not a good thing.

But, in reading Scott's comment, for me personally I also have a very hard time with the heat, but I'm OK till it's 90+ and I'm not doing too much. I have to always make sure I have some sort of drink around. Low sugar, but with calories.

For me its the heat. I do not do well in the heat and am susceptible to heat exhaustion really badly because I sweat so much. So once summer is in full spring, I have to do anything outside in the early morning before it hits the 80 degree mark pretty much.

Two words: goose poop. It seems no matter what park we go to locally, there are geese EVERYWHERE! Don't get me wrong; I love the geese and think they are adorable. But they poop everywhere I want to sit! LOL

Hunger and bathroom breaks are our biggest challenges on outings. The biggest problem I have with my teens is they forget to eat. I tend to assume they are 14 and 17, I don't have to remind them to eat before we go somewhere - but yes, I do need to remind them every time.

Actually, hunger is the number one issue. We never seem to pack enough to eat.

I would have to say the thing that ruins trips out such as hiking, biking, car trip, etc is either hunger or having to pee...15 minutes after being asked and 10 miles from the nearest gas station.
